Which plans are being discontinued?

The Plus Plan, the Preferred PPO and the Traditional Plan

What happens to membership on the old plans?

Membership enrolled on plans which are being discontinued have been given 90 days notice and a plan change election form with alternatives effective 7/1/07 on a guaranteed-issue basis. We will send a reminder by 5/31/07 to those who have not responded and advise that they be converted to the plan with the closest benefit match as of 7/1/07 if no reply is received in order to prevent an inadvertent lapse in coverage.

What happens to their deductible accumulation when they change plans?

Charges which have been applied to their deductible will rollover to their new plan with the renewal and continue to accumulate on a Plan year, based on effective date.

What happens to their existing dental plan upon renewal?

The dental will continue without interruption, and with a rate hold from last year.

When will the new plans become effective?

The renewal changes will become effective 7/1/07 for all individual plan members.

Is there a new application and when do I have to stop using the current one?

YES, there is a new application. You must use the new application for any applicant signature obtained after 7/1/07 or if the new plans are being selected. The current application can be used if signed by the applicant before 7/1/07 and they are choosing one of the old plans.

When will the new brochure and application be available?

When will the website be updated with the new benefits, rates and material?

Now. The website includes the new plan information and PDF of the brochure and application for plans effective 7/1/07.
